    Following is a list of common blank forms used for Information Systems. They are intended to be accessed using Adobe Acrobat for both viewing and printing. Click on the link provided to view each form as needed and print by sending the print to any printer you have available to your workstation.

     
     
     
    Ethnicity/Race Questionnaire

    This form is intended to be completed by a parent/guardian of every child at the first time the child is enrolled to attend school at ECISD. It is also intended to be completed by every employee of the district at the time he or she is first employed by the district. The form does not need to be completed again unless the completed form is lost. The completed form should be retained either in the permanent record of the student or, in the case of staff, in the employee’s Human Resources file.

    Public Information Request
    Public information
     
    This form is intended for use by persons requesting information which may be released under the Freedom of Information Act. "Public Information" is information that is collected, assembled or maintained under a law or ordinance or in connection with the transaction of official business by the Board or for the Board and to which the Board has a right of access (Gov't Code 5552.002(a)).

    Public information requests are subject to fees. A cost estimate will be provided prior to the processing of the request.

    Student Directory Information Release

    This form is intended to be completed by a parent/guardian of every child at the time the child is enrolled to attend school. The completed form is the official record for the authorization to release directory type information for the student to the public. The completed form should be retained in the student's permanent record.

    Use Agreement

    This form is intended for all users of the iTCCS online database system. Use this form to identify all new users and new or changed teaching staff. Return the completed forms to Information Systems to obtain login information for access to the Student, txGradebook, or Business applications.

     
    Online System Access Request

    This form is intended for all users of the Online Business and Student Databases. This includes access by means of the Passport e-Client program, commonly referred to as Passport, and by means of the iTCCS application programs, commonly referred to as web-apps, which operate web-based using the internet web browser, Internet Explorer.

    Online Access forms must be approved by the campus principal or administrative department manager and returned to Information Systems to obtain login information for access to the online databases or to obtain authorization for additional online functions for existing users. The form has been designed to be filled electronically and then either printed and physically delivered (mail or fax) to Information Systems or submitted electronically via e-mail. When submitted electronically, it may be desirable to retain either the send copy of the e-mail or a printed copy as a record. If the request is initiated by someone other than the approving principal or administrator and it is being submitted electronically, it should first be e-mailed to the approver, who can then forward it to Information Systems. The e-mail delivered to Information Systems must be sent from the approving principal or administrator.

    Discipline Verification

    This form is intended for use by the Discipline/Data Clerk as a worksheet in the processing of student discipline data every six-week cycle. Return the completed forms to the Discipline Specialist in Information Systems.

    End of Year Attendance Instructions

    This form is intended for use by the campus Attendance Clerk. It details all of the steps that must be completed at the end of the school year for balancing attendance and packing the attendance audit box. This document is for clerk use only and need not be filed in the PEIMS Documents Audit Box or returned to Information Systems.

    PEIMS Documents Checklist

    This form is intended for use by the campus Attendance Clerk. It is a checklist of all the documents that must be placed in the PEIMS Documents Audit Box. This checklist is completed by the Attendance Clerk and Information Systems staff and filed in the PEIMS Documents Audit Box at the end of each school year.

    PEIMS DAB 16-17
     
    This form is intended for use by the campus Attendance Clerk. Every campus must complete this form and a copy must be on file in Information Systems at the end of each school year. It contains information regarding the storage location of the PEIMS documents audit boxes.
    Important Contents Form
    Important Content 16-17
     
    This form is intended for use by the campus Attendance Clerk. Print and use this label on all boxes containing your annual PEIMS documents audit information. When printing this form, remember to set the printer to landscape format. Write or print on the label what the contents of the box are.
    Student At-Risk Profile

    This form is intended to be completed by the campus staff responsible for PEIMS reporting of at-risk students. Use this form to document students at-risk of dropping out of school who do not have a pre-printed At-Risk Profile for the current school year. This completed form is kept in the student's pemanent record folder, and, as such, needs to be printed on yellow paper.
    Not Highly Qualified Teacher Letter Request

    This form is intended to be used by campus administrative staff. It is required that this form be submitted in order to request the production of parent notification letters on the non-highly qualified status of teaching staff. Please note that answers to all but the last question are required. Completed forms may be sent to Information Systems by any convenient method.
    Attendance and Homebound Checklist

    This form is intended to be used by the campus attendance clerks. It is required that this form be submitted to the appropriate support analyst in Information Systems at the end of every attendance reporting cycle. Completed forms may be sent to Information Systems by any convenient method.
    Bullying Checklist
    1
     
    This form is intended for use by administrators responsible for evaluating an allegation of harassment/bullying. It can be completed online and saved to your local hard drive for archiving purposes.
